Telephone: Focused reinforcement of the discharge plan with advanced practice nurse (APN) telephone follow up on the day of discharge, day 3 after discharge and weekly for 4 weeks to assess post discharge status in the areas of activity level, assistance needed, reinforce the discharge home plan, and assess for complications.
