Quality level was based on the OCEBM Levels of Evidence Scale from 1-5, wherein level 1 represents the highest quality evidence and level 5 represents the lowest quality evidence. Overall grade of recommendation utilized a scale from "a" to "d" wherein "a" reflects consistent level 1 studies and "d" reflects consistent level 5 studies or troublingly inconsistent or inconclusive studies of any level.
