Validity of Two High-Incline VO2max Protocols for College- Aged Population

DE Lankford, FACSM, Jake T Bartschi, Keegan Huntsman, Lex Gidley, Tyler Hook and Yilin Wu

Article Type: Research Article | First Published: November 04, 2019

The aim of the study is to validate two high-incline Graded Exercise Tests (GXT) that could be used as alternatives to the Bruce protocol to obtain VO2max values for college-aged individuals who may not be accustomed to running on a treadmill. Maximal oxygen uptake (VO2max) assesses the upper limit of an individual's ability to consume and utilize oxygen during intense, or maximal exercise, commonly accepted as a reliable indicator of cardiorespiratory health. Among all the existing protocols, Graded Exercise Tests (GXT) that require direct measures of oxygen uptake through continuous gas anal...

Age and Incidence of Pharyngeal Candidiasis in Patients Undergoing Bronchoscopy

Victor Omiwole, Sardar Ali and Junaid Ali

Article Type: Original Article | First Published: November 09, 2019

Candidiasis is a type of fungal infection that often spreads to the skin, mouth, throat, esophagus, and vagina. It is caused by the build-up of yeast called Candida. There are multiple types of Candida that can be infective agents, with C. albicans being the most prevalent. When it spreads to the mouth and throat, it is called thrush or otherwise known as Oropharyngeal Candidiasis. In this retrospective study, the frequency of candidiasis in the throat (pharyngeal), was examined exclusively in 1255 patients who underwent bronchoscopy. Most of the findings of pharyngeal candidiasis were inciden...

Steroid Induced Psychosis in a Rare Case of Bronchiolitis Obliterans Organizing Pneumonia

Bryn Dhir and Rachit Kumar

Article Type: Case Report | First Published: November 07, 2019

We report a case of an 84-year-old female with altered mental status and known Bronchiolitis Obliterans Organizing Pneumonia suspected from high dose steroid who presented for medical management of acute psychosis. This case centers on the inpatient management of older patients in the setting of steroid use with consideration given to dosing, supportive care and therapy during management. Iatrogenic psychiatric conditions due to therapeutic medical management of pulmonary diseases can be complicated when options for medical management is limited. This may be further complicated with increased ...

A Rare Cause of Respiratory Distress in Infants: Double Aortic Arch

Tolga Bacak, Sebnem Ozdogan, Seda Geylani Gulec and Evrim Ozmen

Article Type: Case Report | First Published: October 21, 2019

Vascular ring, which is a rare cause of respiratory distress in infants, results in respiratory and gastrointestinal findings due to tracheal and/or esophageal compression, but the diagnosis may be delayed because of the nonspecific symptoms. Double aortic arch is the most common vascular ring anomaly. A case of double aortic arch presented with respiratory distress, stridor, and feeding intolerance during infancy is discussed. Vascular ring is seen with an incidence of 1% in all congenital cardiovascular anomalies. Double aortic arch is the most common anomaly among vascular ring pathologies ...

Correlation of Pathologic and Radiologic Complete Response in the Axilla after Neoadjuvant Chemotherapy for Breast Cancer

Jessica C Gooch, MD, Timothy Borjas, MD, Jennifer Chun, MPH, Elianna Kaplowitz, MPH, Amber Guth, MD, Deborah Axelrod, MD, Richard Shapiro, MD, Linda Moy, MD and Freya Schnabel, MD

Article Type: Original Article | First Published: November 08, 2019

Accurate assessment of locoregional response to neoadjuvant chemotherapy (NAC) is important for surgical planning, particularly in regard to management of the axilla. We evaluated the correlation between radiologic complete response (rCR) in the axilla on magnetic resonance imaging (MRI) and the likelihood of pathologic complete response (pCR) at the time of breast cancer surgery following NAC. Our institutional database was queried for patients who underwent NAC from 2010-2016 and had a preand post-NAC MRI performed. Clinicopathologic characteristics and MRI findings were recorded and correla...

Study on the Variations of Image Density Index of MDCT for Healthy Lungs

Won-Bin Cha, Sok-Chan Yun, Chan-Ung Ri and Chol-Ho Chang

Article Type: Original Article | First Published: October 21, 2019

We selected several image density indices to establish CAD for prediction of biopsy in lung diseases. Our results suggested that significant difference was not observed in the image density indices between bilateral lungs at each slice level. Our own designed image indices may be reliable parameters in the establishment of CAD system and pathologic diagnosis for lung disease. There has been a rapid progress in the modality of CT since it was developed by Mr. Hounsfield. The earlier CT had been upgraded to the helical CT and for the time being the MDCT is widely used in the radiology practice. ...

Disseminated Cryptococcosis in a Deceptively Immunocompetent Adolescent

Vivek Sood, Raja Ramachandran, Rakesh Kumar Pilania, Arun Prabhakar, Neeraj Inamdar, Navin Pattanashetti, Vibhu Joshi, Madhubala Sharma, Amit Rawat, HS Kohli and KL Gupta

Article Type: Case Report | First Published: October 28, 2019

Cryptococcosis particularly if disseminated or extrapulmonary, in an otherwise immune-competent individual, especially if young, requires thorough evaluation for underlying primary immunodeficiency including Mendelian susceptibility to mycobacterial disease (MSMD). In this context, we describe an adolescent diagnosed with disseminated cryptococcosis involving skin, lymph nodes, spleen and hepato-biliary involvement with reduced IL-12β1 receptor expression suggesting underlying MSMD. Following initial management with conventional amphotericin and flucytosine, along with antibiotics for cholang...

Widespread Multi-Organ Involvement by Lymphoplasmacytic B-Cell Lymphoma in a Patient with Multiple Comorbidities: A Case Report

Oluwaseyi Olayinka, Gagandeep Kaur, Nusrat Pathan and Paul Fiedler

Article Type: Case Report | First Published: October 31, 2019

Lymphoplasmacytic lymphoma (LPL) is an uncommon B-cell non-Hodgkin lymphoma in which the affected tissue, usually bone marrow, is infiltrated by small lymphocytes, plasmacytoid lymphocytes, and plasma cells. The lymph nodes, spleen and other tissues/organs in the body are less commonly infiltrated by the neoplastic cells. The clinical course of LPL is indolent although some studies report a more aggressive course and adverse outcomes especially in patients with other primary malignancies including skin cancers. We report an unusual finding of widespread multi-organ involvement by LPL in a 69-y...

Dina Tulina, Mike KS Chan, Michele WBF and Shing Yi Pan

Article Type: Research Article | First Published: November 04, 2019

Immune Modulating Peptides (IMP) discussed in literature as a universal immunocorrectors with wide areas of usage which balance the immune system without causing global immune suppression or overreaction. The present article aim is to review the effect and drawbacks of IMPs in form of Dialyzable leukocyte Extracts (DLE) or Transfer Factors (TF) during and following chemotherapy, radiotherapy, drugs interactions, cancer response and clinical outcomes in different cases of cancers such as metastatic breast cancer, glioma, prostate cancer, osteosarcoma and others. The considered cases of immune o...
