Modified Rhinolift: A Minimally Invasive Technique for Age- Related Tip Ptosis

Nikolaus Hjelm, MD, Peter Ciolek, MD, Ryan Heffelfinger, MD and Howard Krein MD, PhD

Article Type: Case Report | First Published: October 07, 2019

As one ages, the structure of the nose undergoes a number of changes, leading to alterations in form and function. Patients with no previous history of nasal issues may present with age-related nasal obstruction later in life. Tip ptosis is a well-recognized etiology of nasal obstruction in the aging patient. The pathogenesis is believed to be due to weakening of several of the tip support mechanisms. Of primary importance, there is age-related degradation of the fibrous union at the cephalic edge of the lower lateral cartilage and the caudal edge of the upper lateral cartilage known as the sc...

The Efficacy, Safety and Side-Effect Profile of the Ketogenic Diet in the Treatment of Pediatric Super-Refractory Status Epilepticus

Gurkan Gurbuz, Burcu Kumru, Sevinc Ozsevik, Ali Evren Bilgic and Aycan Unalp

Article Type: Research Article | First Published: October 16, 2019

Four of the six patients (66.6%) were female. Patients’ mean age was 2.8 years (min: 1, max: 9 years). Four epileptic encephalopathies were determined, two Lennox- Gastaut, one WWOX encephalopathy, and one undiagnosed epileptic encephalopathy. Mean time from the start of status epilepticus to start of ketogenic diet was 13.1 days (min: 8, max: 20) and mean duration of the ketogenic diet was 4.1 months (min: 1, max: 8). One patient's seizures ceased, and the seizures of two other patients decreased by more than 50%. One patient died from complications not associated with the ketogenic diet. I...

Design and Validation of a Survey for the Study of Food and Nutritional Security to Climate Change Vulnerability

Carlos Matias Scavuzzo, Eliana Álvarez Di Fino, Alberto Ferral and Maria Daniela Defago

Article Type: Original Article | First Published: October 14, 2019

Human Security is a concept instituted by the United Nations and refers to the possibility that each person can satisfy basic needs of different kinds: Health, food, environment, education, economy, politics, institutional access, etc. Both WHO and FAO emphasize these concepts related to food and nutritional security (FNS), and relate it to the vulnerability derived from emergency and disaster situations. These are increasingly frequent and complex in a context of climate change, since they affect food systems, directly and indirectly impacting the pillars of FNS: availability, economic access...

Interventions for Treating Urinary Incontinence in Residential Care: A Narrative Review

Karen Farrell and Akke Vellinga

Article Type: Narrative Review | First Published: October 18, 2019

Urinary incontinence (UI) is highly prevalent in elderly people residing in nursing homes. Care staff often lack the knowledge and management skills to effectively care for and treat residents with UI. The implementation of standardised treatment interventions at the individual or nursing home level can help to successfully manage and reduce UI in elderly residents. MEDLINE and PubMed were searched from 2005-2019 using selective search strategies, detailing interventions and randomised controlled trials (RCTs) in residential care and nursing homes, focusing on elderly people. Pharmacological a...

Successful Treatment of Hepatitis C, Genotype 3, Treatment Failure (Sofosbuvir/Daclatasvir) with Sofosbuvir/Velpatasvir in Decompensated Cirrhosis Complicated by Renal Insufficiency

Marco Distefano, Lorenza Di Marco, Enzo Scifo, Salvatore Gruttadauria, Gaetano Scifo and Vito Di Marco

Article Type: Case Report | First Published: September 30, 2019

Clinical trials and real word data have proven that chronic hepatitis C (HCV) can be eradicated (sustained virological response SVR or ‘cure’) in the majority of patients by direct - acting antivirals (DAAs) . There are, however, groups of patients in whom HCV treatment outcomes with direct acting antivirals (DAAs) are suboptimal (genotype (GT) 3 patients, decompensated cirrhosis, renal failure) or have not been studied in large cohorts (patients with relapse to a previous DAAs treatment (failure)). This case outlines the successful eradication of GT-3 hepatitis C (HCV) in a patient with d...

Profile and Spatialization of Pregnant Women with Gestational Syphilis and Congenital Syphilis in a Municipality in South Brazil

Elenir Terezinha Rizzetti Anversa Daros dos Santos, Nicassia Cioquetta Lock, Sandra Trevisan Beck, Luciane Silva Ramos, Valeria Parecida da Rosa and Teresinha Heck Weiller

Article Type: Research Article | First Published: October 07, 2019

Congenital Syphilis (CS) is one of the most serious adverse outcomes in pregnancy, avoidable through early detection and treatment of the pregnant woman and her sexual partner. During this period, 128 cases of GS and 73 cases of CS were notified. The frequency of notifications during the studied period increased from 2.3% to 39.2% for GS and from 1% to 41% for CS. Only 23.4% of the pregnant women’s partners with GS were treated during antenatal care. Considering the number of GS and CS cases notified by administrative region, the greatest percentage occurred in the West region of the municip...

Patient and Provider Communication Regarding Exercise during Pregnancy in a Rural Setting

Maire Blankenship, Kim Link, Samantha Henry and Rachel Tinius

Article Type: Original Research Article | First Published: October 07, 2019

Women in rural settings are at increased risk for adverse pregnancy outcomes. One potential way to improve pregnancy outcomes in rural settings is through physical activity promotion. However, given the disparities in prenatal care, women in rural areas may not receive information from their health care provider regarding physical activity during pregnancy. Therefore, the purpose of this study was to examine patient and provider communication in a rural setting (from both patients' and providers' perspectives) regarding physical activity during pregnancy. A mixed methods study was performed in...

The Effect of Concurrent Exercise Order (Resistance and Endurance) on Lipid Profile, Leptin Serum and Insulin Resistance Index in Overweight Women

Mehdi Hakimi and Maryam Ali-Mohammadi

Article Type: Research Article | First Published: October 17, 2019

The implementation of a concurrent exercise session in two exercise groups except for the indicators of HDL that there was no significant change, caused to significantly decreases in TC, TG, LDL, insulin, insulin resistance indices and leptin serum that this rate of changes was significant compared with the control group. But no significant difference was observed between implementation of two methods of concurrent exercise. In addition, the control group did not change significantly in none of the variables. Although the implementation one session of a combination of resistance and endurance ...

Limbus Vertebra and Low Back Pain: A Case Report and Review of Literature

Man R Shim, MD

Article Type: Case Report | First Published: October 14, 2019

Limbus vertebra is marginal interosseous herniation of the nucleus pulposus. It is generally considered to be an incidental finding without any symptoms unless posterior herniation causes nerve compression. However, recent findings have suggested limbus vertebra may be associated with a specific gene and intervertebral disk degeneration. This article reports a case of a 40-year-old male patient with limbus vertebra, intervertebral disk degeneration, and low back pain, followed by a comprehensive literature review. 40-year-old male patient with chronic low back pain presented with an acute exac...

Article Type: Research Article | First Published: October 07, 2019

The trunk muscles have an important role in the stabilization of the trunk in the elderly, however it is not known how much they can influence in the functional performance of this specific population. There were 45 elderly women in the study. The maximum strength and endurance of the trunk and the functional performance were evaluated through the stable wooden seat, McGill protocol, Senior Fitness Test battery and the deadlift. For statistical purposes, a multiple regression model was used. The trunk strength and endurance were used as independent variables and functional performance as a dep...
