Patients with relapsed/refractory Multiple Myeloma (rrMM) and End Stage Renal Disease (ESRD) requiring hemodialysis must receive lenalidomide at a dosage of 5 mg daily on days 1-21 of repeated 28-day cycles, due to substantial elimination via the kidneys. Unfortunately, despite dose reduction, severe adverse events occur with a high frequency. Therefore, lenalidomide is recommended mainly in patients with mild-to-moderate renal impairment....

Arginine is an essential amino acid. This compound plays an important role in various body functions including cell division, wound healing, removal of ammonia, immune function, and release of hormones. The arginase 1 is a hepatic enzyme that is the faulty component in urea cycle. The urea cycle is represented in Figure 1 with focusing on arginine. When in normal function, the enzyme catalyzes the reaction of conversion of the arginine in ornithine and urea. John-Cunningham virus (JCV) has been recognized as a major contributor to progressive multifocal leukoencephalopathy (PML) in humans, especially in immunodeficient patients. While the prevalence rate of PML in human populations could be up to 80%, the occurrence of tumorigenesis is around 0.07%. Thus, we investigated whether crude-oil extracts could synergistically increase the carcinogenesis along with T antigen from JC virus in vitro. A young woman presented with an asymptomatic recurrent right sided pleural effusion. Pleural fluid full report showed neutrophilic and haemorrhagic effusion. Adenosine deaminase level in the pleural fluid was elevated and the Mantoux test was positive.
