Characterization of the Olfactory Pathway by Anisotropic Diffusion Using Nuclear Magnetic Resonance Imaging in a Pediatric Population

Hector Ramirez-Flores, Eduardo Barragan-Perez, Pilar Dies-Suarez, Diana Platas-Neri, Julio Garcia and Silvia Hidalgo-Tobon

Article Type: Research Article | First Published: December 09, 2019

One of the techniques of Nuclear Magnetic Resonance is Diffusion Tensor Imaging, which measures the speed of diffusion of extracellular water molecules found in tissues. Diffusion tensor imaging allows virtual dissections of functional white matter tracts in the human brain in vivo using regions of interest (ROI). The intrinsic trajectories of the olfactory pathway that can be identified using the diffusion- weighted magnetic resonance technique provide an important anatomical reference for the evaluation of clinical disorders commonly associated with the olfactory system in the brains of pedi...

Acute Lyme Arthritis Presenting in Bilateral Hips of an Elderly Patient: A Case Report

Lawrence M Asprec, Mitchell K Freedman, Jeffrey A Gehret and George W Young

Article Type: Case report | First Published: December 09, 2019

A 70-year-old male presented with a chief complaint of 10 days of lower back and bilateral lower extremity pain. He described sudden onset of intense, sharp bilateral anterior thigh pain radiating to the medial calves intermittently and without paresthesia, bowel or bladder incontinence, fevers, rashes or weight change. His gait was found to be antalgic and slow. Strength, sensation, and reflexes were intact in both legs. X-ray imaging of bilateral hips revealed mild osteoarthritis bilaterally. An MRI of the bilateral hips showed bilateral hip effusions with synovial proliferation and degenera...

Isolated Neuropathy of the Motor Branch of the Ulnar Nerve Sparing the Hypothenar Muscle and Ulnar Intrinsic Groups due to Compressive Lesion: A Case Report

Lawrence M Asprec, MD, Mitchell K Freedman, Pedro K Beredjiklian and George W Young

Article Type: Case report | First Published: December 09, 2019

A 57-year-old man developed progressive weakness over 3 months in his right hand without sensory abnormalities. Clinical and electrodiagnostic examination revealed abnormalities in the FDIO. Clinical examination supported an abnormal AdP. Hypothenar muscle groups were intact, suggestive of an ulnar nerve lesion in the palm, an uncommon area of entrapment. MRI confirmed a ganglion cyst between the third and fourth metacarpal bases, compressing the deep palmar branch of the ulnar nerve. After removal of the cyst and neurolysis of the ulnar motor branch, AdP and FDIO strength improved five months...

Importance of Genetic Polymorphisms (Rs662 and Rs854560) of the Paraoxonase-1 Gene as Risk Factor for Acute Ischemic Stroke in Turkish Population

Nevin Karakus, Nursah Basol, Serbulent Yigit, Pinar Cogas, Tuba Ceviz, Ali Kablan and Osman Demir

Article Type: Research Article | First Published: December 11, 2019

Allele frequencies of PON1 gene Q192R and L55M polymorphisms didn’t demonstrate any significant differences between patients and controls (p > 0.05). However, genotype frequencies were significantly different between patients and controls in terms of Q192R polymorphism (p = 0.030). Statistically significant correlations were observed between systolic and diastolic blood pressures of patients and L55M polymorphism (p = 0.017 and p = 0.039, respectively). Heterozygosity for the both Q192R and L55M polymorphisms demonstrated a statistically significant association with acute ischemic stroke in ...

Two Unique Cases of Takotsubo Syndrome: Asymptomatic Takotsubo Complicated by Massive Left Ventricular Thrombus and Nightmare Induced Takotsubo Syndrome

Robert W Murdock, DO, Todd Thomas, DO and David K Murdock, MD

Article Type: Case Report | First Published: December 13, 2019

Takotsubo syndrome (TTS) typically presents like an acute coronary syndrome and is commonly associated with acute physical and/or emotional stress. TTS is characterized by transient moderate to severe left ventricular (LV) dysfunction in the absence of obstructive coronary artery disease (CAD). Excessive beta adrenergic stimulation remains the most widely accepted theory to account for development of TTS. Importantly a variety of commonly used pharmacologic agents which enhance catecholaminergic activity have been recognized as causing or facilitating TTS....

Prevalence of Antiphospholipid Antibodies in HIV-Infected Children in Ile-Ife, South-Western Nigeria

Oyelese AT, Salawu L, Adejuyigbe EA and Olasanmi O

Article Type: Review Article | First Published: December 12, 2019

Lupus anticoagulant (LA) and anticardiolipin antibodies (aCL) are acquired autoantibodies referred to as antiphospholipid antibodies (aPL). They can be found in infective and non-infective conditions. Infection with the Human immunodeficiency virus (HIV) can predispose patients to having these antibodies and this has been documented in Nigerian adult patients but not in the pediatric age group. To determine the prevalence of antiphospholipid antibodies in HIV-infected children and its association with the clinical stage of infection and possible hematological derangements were the aim of this ...

Anaesthesia Monitoring using Artificial Intelligence Techniques

Harry Mc Grath, Colin Flanagan, Liaoyuan Zeng and Yiming Lei

Article Type: Original Article | First Published: December 07, 2019

Artificial Intelligence (AI) will change our lives beyond recognition, it will have a far bigger impact than the internet or mobile technology. We are now at the threshold where machine intelligence is comparable with human intelligence, in certain limited aspects, for the first time in history. Enhancements in technology, both software and hardware, have resulted in some human decision-making being inferior to, and more erratic than, AI in many fields, including medicine. However, it is widely accepted that rather than compete with machines, using AI to support and help make better informed d...

Nephrocutaneous Fistula after Microwave Thermal Ablation and Literature Review

Marius Anglickis, Giedre Anglickiene, Gintare Andreikaite, Lina Miklyciute and Edmundas Starolis

Article Type: Case Report | First Published: December 13, 2019

Nephrocutaneous fistula is a very rare complication of renal surgery. It may be associated with renal tumors, nonfunctional kidneys, staghorn calculi, renal trauma or chronic urinary tract infections. Because of widely used imaging techniques an early detection of small renal tumors has modified the management of kidney cancer. Nephron sparing surgical resection remains the standard of care for T1 tumors less than 7 cm. For patients who are not amenable for surgery, microwave thermal ablation (MWA) is gradually becoming a preferred treatment modality. In this article we present a rare complica...

Article Type: Case Report | First Published: December 14, 2019

The odontogenic keratocyst (OKC) is a benign pathology that presents characteristics of aggression and local recurrence. The WHO considered it as an odontogenic tumor in 2005, but in the most recent classification in 2017 it was once again considered as an odontogenic developmental cyst. Most of the time it presents a symptomatically, but the patient can report the pain symptomatology. The aim of this article is to report a clinical case of OKC on maxilla describing its characteristics and forms of treatment. Male patient, 15-years-old, referred to the bucomaxillofacial surgery service of Hosp...
