The Sociodemographic Characteristics of Patients Who Experience Postdischarge Adverse Events

Vishal Dahya and Jessica Bishop-Royse

Article Type: Research Article | First Published: June 16, 2018

In the decade since the publication of Institute of Medicine reports on patient safety, much remains unknown about the patient-level characteristics that may increase or decrease vulnerabilities to postdischarge adverse events. We reviewed the patient safety literature to determine whether specific sociodemographic groups are vulnerable to postdischarge adverse events. We found substantial inconsistencies in how postdischarge adverse events were defined, which was largely driven by innovations and developments in research methodology that have occurred since the publication of the IOM reports....

Age-Related Cognitive Changes: The Importance of Modulating Factors

Rut Correia, Jose Barroso and Antonieta Nieto

Article Type: Review Article | First Published: June 16, 2018

The study of age-related cognitive changes over the last years reveals a considerable variability in available results. The methodological heterogeneity observed in the aging field can significantly contribute to this observed variability, which creates a confusing scenario for beginners in this topic. The main aim of the present article is to extract a clear depiction of the current state of knowledge about age-related cognitive changes regardless of the methodological approach followed by researchers....

Performance of Hi-Flex Femoral Component in Total Knee Arthroplasty - A Randomized Control Study

Sheldon Moniz, Ryan Du Sart and Piers Yates

Article Type: Research Article | First Published: June 16, 2018

Total knee replacement (TKR) is a surgical procedure for intractable degenerative disease of the knee joint. Despite continued evolution in prosthesis design and surgical procedures, restricted range of motion and functional performance is still common in patients undergoing TKR. Posterior stabilized TKR aims to maintain a more reproducible roll back than cruciate retaining and mobile bearing knees, possibly leading to better flexion and function....

Multifocal Avascular Necrosis of the Lunate and Triquetrum in a Child with Systemic Lupus Erythematosus

Roberts DC, Jester A, Southwood T, Johnson K and Oestreich K

Article Type: Case Report | First Published: June 15, 2018

Avascular necrosis is a known complication of systemic lupus erythematosus. We report an unusual case of avascular necrosis affecting both the lunate and triquetrum in a child with this condition. Vasculitis, synovitis and IgM anticardiolipin antibodies were probable predisposing factors. The use of arthroscopic synovial debridement improves symptoms even in the presence of carpal chondromalacia and potentially delays the need for salvage surgery....

Autoimmunity and Lymphoma: A Brief Review

Edward B Miller

Article Type: Review Article | First Published: June 15, 2018

The relationship between the immune system and both Hodgkin's (HL) and Non-Hodgkin's (NHL) lymphomas is a complex bidirectional process which has fascinated researchers and clinicians for many years. Lymphomas of all types are known to be associated with autoimmune paraneoplastic manifestations, and conversely are recognized in increased frequency in patients with pre-existing autoimmune diseases....

A Giant Cell Granuloma of the Temporal Bone: A Case Report

Giyas Ayberk, Selen Isık Adiloglu, Aylin Kılıc Yazgan, Mahmut Ferhat and Mehmet Faik Ozveren

Article Type: Case Report | First Published: June 15, 2018

A giant cell granuloma (GCG) rarely occurs in the skull bones. Although it has destructive character, the GCG is used as the synonym of giant cell reparative granuloma (GCRG) in the literature. Differentiation from other osteolytic lesions quite difficult. The first case of GCG in the temporal bone was reported by Hirschl and Katz in 1974. Even though the GCG is not a true neoplasm, the locally aggressive behavior of this tumor necessitates surgical excision whenever possible....

Impact of the Biomechanical 'Pleural Stress' Pressure Vessel Hypothesis on the Pathogenesis of Tuberculosis: Predictors of TB Reactivation and its Application to TB Screening

Aaron R Casha, Roberto Caruana-Gauci, Alexander Manche, Marilyn Gauci, Ruben Gatt, Hannah-Marie Casha-Gauci and Joseph N Grima

Article Type: Review Article | First Published: June 15, 2018

A hypothesis for the pathogenesis of pulmonary TB is presented based on the biomechanics of pleural stress in the context of the lung as a pressure vessel. This hypothesis modelled pleural stress as a marker of pleural tissue destruction and subsequent repair. It identified five predictors of TB reactivation, namely adolescent and young adult age, male gender, apical location, low antero-posterior chest diameter or low thoracic-index rib-cage shape and the presence of pre-existing apical blebs....

Diffuse Idiopathic Pulmonary Neuroendocrine Cell Hyperplasia (DIPNECH) - A Case Report of Successful SSA Therapy after Failure of Systemic Corticosteroids

Maja Badovinac, Matevz Harlander, Branislava Rankovic and Marjeta Tercelj

Article Type: Case Report | First Published: June 15, 2018

Treatment of diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH) has not been established yet. The majority of patients is treated with glucocorticoids. This article presents a case when such treatment was inefficient. When switched to somatostatin analogues treatment, patient's clinical condition and radiologic findings improved. Nonetheless, we were not able to prove the expression of somatostatin receptors in the lung nodules....
